1. Your initial appointment will consist
of a consultation explaining your diagnosis and treatment options.
Occasionally, treatment can be done the same day as the consultation;
however it is common in our practice to have a phone conference
with the patient and the referring doctor prior to that visit.
In most instances a complete evaluation and treatment plan visit
is required prior to the actual treatment.
For the more comprehensive treatment plans the initial visit
may be a dedicated to collecting all the diagnostic data, such
as periodontal charting, x-rays, dental scans etc… Your
treatment plan will be discussed after a meeting with the other
members of your dental team. 2. Please
provide the following information at the time of your consultation:
o Your referral slip and X-rays if applicable
o A list of medications you are presently taking
o If you have dental insurance, bring the necessary completed
forms. We will provide you with the appropriate ADA codes,
so you can file your claim as quickly as possible.
3. Important: A parent or guardian must accompany
all patients under the age of 18 at the consultation.
4. Please alert the office if you have
a medical condition that may be of concern prior to surgery
(e.g. diabetes, high blood pressure, artificial heart valves
and joints, rheumatic fever, etc.) or if you are on any medication
(e.g. heart medication, aspirin, anticoagulant therapy, etc.)
